Raman histology matches H&E in most pituitary samples; nucleus monitoring tracks hearing in schwannoma surgery; Siemens joins BCI imaging effort

An operating microscope on a wheeled stand with an articulated boom arm, its binocular head angled down over an empty table.

A Journal of Neurosurgery study found that stimulated Raman histology produced images with excellent comparability to conventional histopathology in 37 of 66 pituitary tumor samples during endoscopic transsphenoidal surgery. A second study tied dorsal cochlear nucleus monitoring to hearing after vestibular schwannoma surgery, and Paradromics and Siemens Healthineers are collaborating on a brain-computer interface imaging platform.

A Journal of Neurosurgery study compared stimulated Raman histology (SRH) images with conventional hematoxylin and eosin (H&E) histopathology in 66 tissue samples from 28 adults operated on for suspected pituitary neuroendocrine tumors during endoscopic transsphenoidal surgery. SRH showed excellent comparability in 37 samples (56.1%), good in 20 (30.3%) and low in nine (13.6%), generating near H&E-quality digital images within minutes, the authors said. They said future AI integration is likely to further improve diagnostic speed and accuracy.

A retrospective Journal of Neurosurgery study of 153 adults who had retrosigmoid vestibular schwannoma resection used a four-contact depth electrode to monitor the dorsal cochlear nucleus action potential (DNAP). Stable baseline signals were recorded in 80.4% of patients versus 60.8% with auditory brainstem response (ABR) monitoring, and no procedure-related complications were attributable to the electrode. DNAP preservation was the most consistent independent predictor of hearing outcomes, with a residual amplitude ratio cutoff of 0.41 for early outcomes, the authors said.

Paradromics and Siemens Healthineers are collaborating on a multimodal imaging platform for implantable brain-computer interfaces (BCIs) with an end-to-end package for image acquisition, co-registration and processing, Fierce Biotech reported Oct. 19. The workflow is meant to cover candidacy assessment, neural target identification, surgical planning, intraoperative navigation and post-implant localization. "BCI performance begins before an implant ever reaches the brain," said Paradromics CEO Matt Angle.
