The Rural Health Transformation Program sends $10 billion a year to the 50 states for fiscal years 2026 through 2030, under the budget law signed July 4, 2025. Hospitals, clinics and other providers get the money through their state's funding rounds. This tracker follows each state's first-year award, where its plan names AI or other technology, its funding rounds and deadlines, and its awards, each linked to its source. The tracker starts with Colorado; other states are added as they are reviewed.

Where the plan names technology
Initiative 10 of the state's plan, "Expand Rural Telehealth & Technology Integration," covers eConsults, remote patient monitoring, a "statewide technology readiness assessment" and "cybersecurity and interoperability training to rural facilities." The plan's text does not name AI; the program's FAQs say "AI-enabled tools, subscription platforms, and other technology solutions are allowable when they directly support approved project activities."

CMS sets each state's fiscal 2027 amount by Oct. 31, 2026. Colorado's awards are annual and renewable for up to five years, and its FAQs say "the current expectation is that participation requires applying to the RFA issued in June 2026."
