- Initiative 4 of North Dakota's plan, "Connect Tech, Data and Providers for a Stronger North Dakota," would "Enhance existing EHRs with additional technology and AI add-ons," and the plan says "AI will also support rural providers with decision support tools and diagnostics"; its Clinics Without Walls strategy invests in "a statewide telehealth network" and remote patient monitoring. The state's 2026 rounds include a $1,000,000 Harnessing Artificial Intelligence Innovation round covering predictive analytics and "Agentic outreach, assessments, triage and scheduling," and an EMR enhancement round for "technology and AI add-ons" and cybersecurity.

- North Dakota's funding page labels the Zero-Hour PE, Community Gardens and Behavioral Health Promotion rounds "Awarded" (23, 45 and 79 applicants) but names no recipients, and no state announcement of subaward recipients was found. That page is too long to be read in full for this tracker, so rounds listed after the Behavioral Health Promotion grants are shown as closed from their deadlines. CMS's Aug. 20 release describes the $1 million Coordinating and Connecting Care Initiative as launched. The program page says "North Dakota is expected to receive $1 billion over five years."
