High-volume injections fail to beat sham for Achilles tendinopathy; FDA alert warns of fire risk as Medline exits bed market

High-volume injections, with or without a corticosteroid, did no better than sham at 12 months for midportion Achilles tendinopathy in adults also given exercise, a 192-patient single-center trial in The BMJ found. The Food and Drug Administration (FDA) posted an early alert on fire and entrapment risks with all Medline beds, including homecare beds, after two deaths and 16 serious injuries.

In the single-center Inject trial, 192 adults with midportion Achilles tendinopathy were randomly assigned to sham or to high-volume injections with or without corticosteroid, all with the same exercise program and education, according to The BMJ. At 12 months, VISA-A differences between sham and the injections were -3.5 (98.33% CI -12.0 to 5.0) with corticosteroid and 4.0 (-4.2 to 12.3) without. The corticosteroid injection beat sham at six weeks (9.2, 1.5 to 16.9) but trailed the injection without corticosteroid at 26 weeks (-8.6, -16.4 to -0.7). The authors said the data do not support routine use of the corticosteroid injection.

The Food and Drug Administration (FDA) posted an early alert after Medline notified customers Oct. 14 of the discontinuation and market exit of all Medline Brand beds, including homecare, bariatric and long-term care beds. Overheating of the electrical components may progress to fire, and there is a potential risk of entrapment or asphyxiation, the FDA said. As of Oct. 20, Medline had reported two deaths and 16 serious injuries. The FDA said users should unplug the beds except while repositioning a patient and not use extension cords.
